Thank you for signing up to become a Chattanooga Champion as a volunteer with Chattanooga Sports. We greatly appreciate your willingness to help at our events throughout the year. This is an annual waiver of liability where you agree to release Chattanooga Sports and the Chattanooga Tourism Co. of all liability while volunteering at our events. This form is in effect for one calendar year.

In consideration of myself and/or my child for whom I am a lawful parent/guardian, being permitted to assist/volunteer at the event(s). I understand and acknowledge that by signing below I am legally agreeing to the statements in the Chattanooga Sports Volunteer Waiver and Release Form.

I acknowledge and assume all the risks of volunteering in the event(s), including but not limited to the effects of weather, traffic, course conditions, course surfaces, falls, and contact with event participants, volunteers, and spectators.

I release, waive covenant not to sue, and forever discharge the “Released Parties” below, from any and all claims, causes of action, damages, losses, costs, expenses, and liabilities of any kind arising from or connected to my/ or my child volunteering with the event(s) or traveling to or from the event(s). The “Released Parties” include Chattanooga Sports, Chattanooga Tourism Co., all event sponsors, event organizers, event promoters, event producers, event staff, event officials, event directors, any sanctioning body, administrators, contractors, vendors, volunteers, athletes all city/state/town governmental bodies or municipal agencies, and all other person or entities involved with the event(s).

I and/or my child agree to adhere to the code of conduct: 1) not to consume any drugs or alcohol that will impair my judgment or ability to volunteer and assist in the event(s). 2) not to speak to the media on behalf of the event. 3) treat all spectators, event participants, and other volunteers with courtesy and respect. 4) listen to the instructions given by event staff, group leaders, and be alert when performing my role.

I authorize medical treatment or care for me and/or my child if advisable in the event of injury, accident, or illness by medical personnel. I agree and assume responsibility for all costs incurred for myself and/or my child not covered by insurance.

I grant Chattanooga Sports and the other “Released Parties” the right and permission to use my and/ or my child’s image, voice, without compensation or further notice captured during the event by Chattanooga Sports, affiliated entities, or contractors, and/or media in all forms, including photographs, video, promotional material, and/or any other record of the event for any purpose whatsoever.

I acknowledge by engaging in volunteer activities related to Chattanooga Sports I do so with knowledge of the risk and potential exposure involved and agree to accept any and all inherent risks to myself and/or my child’s personal health.

I affirm that I am eighteen (18) years of age (or will be on the date of the event) or older, I have read this Waiver and Release of Liability Form, I understand its content, and intentionally and voluntarily sign it. For persons under eighteen (18) years of age, a parent or legal guardian must also sign this agreement.
